Subject: New Performance Planning & Review Form SF 15

Issue Date: May 29, 1997

The new State Employee Performance Planning & Review Form may

now be ordered from Forms Management. The form number is SF 15

(R 5/97). Please place your orders now for the amount you

anticipate using during the next three or four months. The

forms will be in packs of 50. Forms Managment expects to have

them ready for shipment on July 1, 1997.

The Performance Planning & Review Form is also available from

the Civil Service web site at http://www.dscs.state.la.us.

The form has also been distributed on diskette in wordperfect

format to agency trainers. Any of these formats may be used.
