Subject:            New Format for Civil Service Score Notices 

Issue Date:       February 24, 2005 

Early in March 2005, Civil Service will change the format in which candidates’ grade notices are issued.  Plain paper notices will replace the current 9 ½ x 5 ½ self-sealing, carbon copy forms.  The new notices will be printed on standard 8 ½ x 11 paper using a standard printer. 

The Staffing Division reminds Human Resources Officers that falsification of grade notices can occur with any format.  Always check the OPEN system to verify grades.  If you discover that a candidate has submitted a falsified score notice to your office, please notify the Program Accountability Division immediately at 225-219-9437.  See General Circular 1593, Reporting Falsified Credentials, issued December 13, 2004, for details of handling falsified score notices.
