Because of recent changes in record-keeping requirements, the Department of State Civil Service has recently made changes to the SF-10 form. Forms Management has printed the revised SF-10, and it is currently available for use. We will continue to accept old forms.

The revised form is also available in a downloadable web version on our website at www.dscs.state.la.us.
