Subject: New Certificate Forms

Issue Date: January 23, 2002

Beginning January 28, 2002, Certificates of Eligibles will no longer be issued on C. S. 26-C forms. We will be using plain 8 x 11½ paper to print certificates. You will receive two (2) copies; one is for retention in your records and the other is to be returned to Civil Service with actions noted. If you need additional copies for your use, photocopies of the certificate may be easily made.
