   PPR Deadline Dates

   Performance Planning and Review Report, Fiscal Year 2006-2007

   An annual report will be due at the end of each fiscal year. The deadline is set in the rules as July 31 each year.

   PPR Anniversary Date Schedules  These provide the first and last dates an employee's planning session can be conducted, the first and last day the rating session can be conducted, and if applicable, the dates of the re-rating.  Simply locate the anniversary date (in red) of the employee on the appropriate quarterly schedule and these important dates will be provided for that date.

5. DOWNLOADABLE FORMS

   These forms have been provided as a public service and have all been successfully processed on a State computer system. However, we cannot guarantee success for all users, due to the wide variety of browsers/platforms in use. It is still the responsibility of the user to use these forms appropriately and in a manner consistent within these limitations. No actual text of these forms may be changed, and doing so may render them null and void. All relevant deadlines still apply.

   Forms Management now has copies of the revised S.F. 15 (Performance Planning and Review) form available to agencies in need of hard copies of the form. They can be contacted at (225) 219-9570.

             Performance Planning and Review Alternate Form (PPR-A)          

The alternate form is not available from Forms Management.  It is only available online in Adobe Acrobat format.  Users may complete the form and use the “save as” command to keep a copy of the form and its contents.
