Subject:             PPR Annual Reporting Requirements for FY 07-08

 

Issue Date:       July 7, 2008

 

The annual report required by Civil Service Rule 10.12 (c) is due by July 31 each year.  The reporting form is attached to this circular.  It is also available on the link next to this general circular announcement.  The information required on the report consists of the following:

 

1.   The number of anniversary dates for classified employees listed by month from

      July 1, 2007 through June 30, 2008.

 

2.   The number of ratings given in each category.  The six categories are OUTSTANDING, EXCEEDS REQUIREMENTS, MEETS REQUIREMENTS, NEEDS IMPROVEMENT, POOR AND UN-RATED.

 

3.   The number of Requests for Review received from employees with anniversary dates July 1, 2007 through June 30, 2008 and a breakdown of the category and resolution of those requests.

 

4.   Signature of the Appointing Authority.

 

If the total anniversary dates and total ratings numbers do not match, a brief explanation of the discrepancy is requested.  Please do not include in your report those employees who were separated prior to their anniversary date.

 

The deadline for receiving the report is August 29, 2008.
