Update Your Information In DSCS Computer Records
Change Request Form
Use this form to
notify Civil Service of:
- Changes to your name
- Changes to your address
- Changes to the parishes where you are
available to work
- changes to your preferences for either
permanent employment offers only, or for both permanent
and temporary employment offers
You may mail
your request for these types of changes to your records to Staffing Division,
Department of State Civil Service, Post Office Box 94111, Capitol Station, Baton Rouge, LA 70804-9111. You may also fax your
request to the Staffing Division at 225-342-2386, or drop it off in person at
our Baton Rouge Testing and Information Office.

*This form has been provided as a public
service and has been successfully processed on a State computer system.
However, we cannot guarantee success for all users, due to the wide variety of
browsers and/or platforms in use. It is still the responsibility of the user to
use the form appropriately and in a manner consistent within these limitations.
