Change Request Form

Use this form to notify Civil Service of:
  • Changes to your name
  • Changes to your address
  • Changes to the parishes where you are available to work
  • changes to your preferences for either permanent employment offers only, or for both permanent and temporary employment offers

You may mail your request for these types of changes to your records to Staffing Division, Department of State Civil Service, Post Office Box 94111, Capitol Station, Baton Rouge, LA 70804-9111.  You may also fax your request to the Staffing Division at 225-342-2386, or drop it off in person at our Baton Rouge Testing and Information Office.
