Documentation Requirements

 

In addition to the standard documentation required for all appointments, the documentation listed below must be maintained for appointments made using the QUEST process.

 

For all appointments, promotions and transfers agencies must maintain the following records.

§         Appointing Authority approval and certification of compliance with Civil Service Rules

§         Application of appointee and any related documents used to verify qualifications such as transcripts, licenses, certifications, etc.

§         Documentation of who verified the qualifications and supporting documents (license, transcript, certification, etc.)

§         Applicant flow information (race, sex, ethnicity data for all applicants)

 

For appointments made using the Quality Express Staffing “QUEST” process, the agency must also maintain these additional records.

§         Names and scores of all candidates who applied, including appointee.

§         Applications of all who applied.

§         Justification of appointment of a candidate who did not meet “preferred qualifications.”

§         Justification of appointment of a candidate who scored below the 50th percentile on required written examinations.

 

 

QUEST Application Retention Schedule

 

The standard retention schedule will be 90 days after the effective date of the appointment.  The applications of everyone who responded to the announcement must be kept for ninety days whether they qualify or not.  All such applicants must be counted as part of applicant flow and for affirmative action reporting.  The application of the person appointed should be kept as long as the person is an employee.

 

Special retention schedule

When either of the following conditions occurs the applications of all persons who responded to the announcement must be kept for a minimum of one year beyond the effective date of the appointment.  

 

1.   The appointee does not meet preferred qualification OR

2.      The appointee scored below the 50th percentile on both the COAST Behavior Assessment and the COAST Office Skills test.
