14)       Request of Southeastern Louisiana University to establish a policy allowing the agency to pay employees for the attainment of an advanced degree, in accordance with Civil Service Rule 6.16(h), effective August 20, 2008. 15)       Consideration of the following requests to establish or amend Rewards and Recognition policies, in accordance with Civil Service Rule 6.16.1 as follows:

b.     Southeastern Louisiana University – amend policy, effective August 15, 2008.  Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

16)       Consideration of the following requests to establish or amend Optional Pay policies in accordance with Civil Service Rule 6.16.2:

b.     Department of Transportation and Development – amend policy, effective August 20, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

17)       Consideration of the following requests for Optional Pay Adjustments for additional duties, in accordance with Civil Service Rule 6.16.2:

 

a.      Department of Health and Hospitals, Office of Mental Health - for Mr. Hampton P.S. Lea, Mental Hospital Associate Administrator 2, in the amount of $2,706.08, effective August 20, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

 

b.     Department of Health and Hospitals – for a permanent 7% adjustment for Ms. Kathi Richardson, Human Resources Director B, effective June 27, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

           

c.      Department of Wildlife and Fisheries - for extension of a temporary 5% adjustment for Ms. Janis Landry, effective July 1, 2008 through December 31,

2008. Department recommends approval effective July 1, 2008 through December 31, 2008, or until the date the duties are removed, whichever is sooner. THE COMMISSION GRANTED THE REQUEST AS RECOMMENDED BY THE DEPARTMENT.

 

d.     School Employees’ Retirement System – to convert a previously approved temporary 2% adjustment for Ms. Jennifer Champagne, Administrative Assistant 5, to a permanent payment and add an additional 3% (for a total of 5%) adjustment, effective July 14, 2008. Department recommends approval of a 2% permanent adjustment. THE COMMISSION GRANTED THE REQUEST AS RECOMMENDED BY THE DEPARTMENT.

 

e.      Department of Treasury – for a conversion from a 5% lump-sum recurring pay adjustment to a permanent 5% pay adjustment for Ms. Jama Scivicque, State Treasury Fiscal Manager, effective August 8, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

 

f.      Department of Treasury – for a one time lump-sum pay adjustment of $3,600.00 (3.3%) for Ms. Alexis Thompson, State Treasury Debt Assistant Director, effective June 6, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

 

g.     Department of Public Safety and Corrections – Corrections Services – for a lump-sum recurring 10% pay adjustment for Warden Steve Rader, Corrections Warden 4, and Warden Lynn Cooper, Corrections Warden 2, effective June 27, 2008. Department recommends approval as requested effective June 27, 2008, for the duration of the additional duties or until June 27, 2009, whichever is sooner. THE COMMISSION GRANTED THE REQUEST AS RECOMMENDED BY THE DEPARTMENT.

18)       Consideration of the following requests to establish or amend Perquisites policies, in accordance with Civil Service Rule 6.3.1:

 

            a.   Department of Health and Hospitals - establish policy to offset telecommuting       related expenses, effective July 1, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

 

            b.   Department of Natural Resources – amend policy, effective August 20, 2008.       Department recommends approval as requested. THE COMMISSION       GRANTED THE REQUEST.

 

            c.   Office of the Lieutenant Governor and the Department of Culture, Recreation,       and Tourism – establish a Cellular Phone Policy, effective September 1, 2008.       Department recommends approval as requested.  WITHDRAWN.

 

            d.   Department of Public Safety and Corrections – Corrections Services – amend       policy, effective August 20, 2008. Department recommends approval as       requested. THE COMMISSION GRANTED THE REQUEST.

19)       Request of the Division of Administration for a one-year extension of the job appointment of Surveyor 5 in the State Land Office, under the provisions of Civil Service Rule 8.14(a), effective November 21, 2008. 21)       Request of the Department of Health and Hospitals, Office of the Secretary for a five-year extension of the job appointment of Dr. Roseanne Prats, Executive Management Officer 3, Position #50355571, under the provisions of Civil Service

22)       Consideration of the following requests for Special Pay Rates in accordance with Civil Service Rule 6.16(a):

 

a.      Capital Area Human Services District – implement premium pay in the amount of $2.00 per hour, for hours worked, for Social Workers, Professional Counselors, and a Mental Health Program Director who work within the Single Point of Entry Program located at Earl K. Long Hospital, effective August 20, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

 

b.     Capital Area Human Services District – implement premium pay in the amount of $2.00 per hour, for hours worked, for Public Health Nurses 4 who work within the Nurse-Family Partnership Program, effective August 20, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

 

c.      LSU Health Sciences Center-Shreveport – establish premium pay in the amount of $0.50 per hour, for all hours worked by Surgical Technicians 1, and $1.00 per hour, for all hours worked by Surgical Technicians 2, effective August 25, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

 

d.     LSU Health Sciences Center-Shreveport – implement premium pay in the amount of $4.00 per hour, for all hours worked, for Laboratory Information System Coordinator, Laboratory Information System Liaison, and Laboratory Information System Specialist, effective July 1, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.

 

e.      Department of Veterans Affairs – implement premium pay for Psychiatric Aide Supervisors 1, 2, and nursing staff for longevity, management responsibilities, and extra credentials at the Louisiana War Veterans Home in Jackson, effective August 20, 2008. Department recommends approval as requested. THE COMMISSION GRANTED THE REQUEST.
