6)         Request of the Department of Health and Hospitals, Pinecrest Developmental Center, for authority under the provisions of Civil Service Rule 12.5, to suspend Ms. Catina Fulton, without pay, pending criminal proceedings against her.  (Appearances by a representative of the Department of Health and Hospitals, Pinecrest Developmental Center and Ms. Catina Fulton.)   the Commission granted authority to the Department of Health and Hospitals, Pinecrest Developmental Center for Mrs. Catina Fulton to remain on forced annual leave until all leave is exhausted. The Commission asked Pinecrest to appear at the July Commission meeting, with its attorney, to report on the Attorney General’s findings and/or the status of the criminal case. The Commission also suggested that the Department of Health and Hospitals try to find a position for Mrs. Fulton in another DHH facility in the immediate area of Pinecrest Developmental Center.

 

7)         Consideration of the following requests for authority to establish or amend the Unscheduled Absences policy under the provisions of Civil Service Rule 12.6(a) as follows:   (Appearance by representatives of the agencies.)

 

            a.       Louisiana State Board of Nursing - to amend its Unscheduled Absences Policy, effective June 15, 2005.  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

b.       St. Charles Parish Housing - to amend its Unscheduled Absenteeism Policy, effective June 15, 2005.  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

8)         Request of the Department of Environmental Quality under the provisions of Civil Service Rule 5.9 for the renewal of the Dual Career Ladder Policy for the Environmental Scientist series, effective June 15, 2005. (Appearance by a representative of the Department of Environmental Quality.)  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

9)         Request of the Department of State to establish, under the provisions of Civil Service Rule 4.1(d)2, unclassified authority for an assistant to the Secretary assigned to the Help America Vote Act program, effective July 1, 2005 through March 31, 2007.  (Appearance by a representative of the Department of State.)  THE REQUEST WAS GRANTED BY THE COMMISSION.  THE DEPARTMENT OF STATE IS REQUIRED TO GIVE A PROGRESS REPORT AT MID TERM.

 

10)       Request of the Department of Health and Hospitals to establish an Employee Recruitment Incentive Award Program under the provisions of Civil Service Rule 6.16(d), effective June 15, 2005, as follows:  (Appearance by a representative of the Department of Health and Hospitals.)  THE DEPARTMENT OF HEALTH AND HOSPITALS WITHDREW THE REQUEST FROM THE AGENDA.

 

The agency would like to make lump sum payments of up to $350.00 to employees as follows:

 

·        The recruiter will be paid $50 when the recruited employee has worked six weeks or more.

·        The recruiter will be paid $300 when the recruited employee has worked six months or more.

 

11)       Consideration of the following requests for authority, under the provisions of Civil Service Rule 6.16.2, for an Optional Pay Adjustment for additional duties:  (Appearance by representatives of the agencies.)

 

a.         Department of Health and Hospitals for a lump sum optional pay adjustment of 10% for Wesley Jerome Boyd, Information Technology Applications Programmer Analyst 2, for FY 04/05, effective June 6, 2005. THE COMMISSION GRANTED AUTHORITY FOR A LUMP SUM OPTIONAL PAY ADJUSTMENT OF 2% FOR WESLEY JEROME BOYD.

 

b.         Department of Health and Hospitals to rescind the temporary optional pay adjustment in the amount of $235.20 biweekly for Diane Batts, Program Manager 2-DHHR, and to request a lump sum optional pay adjustment in the amount of 7.07% for her, effective June 6, 2005 for FY 04-05.  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

c.         Department of Natural Resources to change the effective date for the base pay adjustment for Matharene Hill, Administrative Assistant 5, from March 2, 2005 to March 7, 2005. THE REQUEST WAS GRANTED BY THE COMMISSION.

 

d.         Department of Natural Resources for a base pay adjustment for permanent additional duties in the amount of 7% for Angelin Morgan, Policy Planner 5, effective June 15, 2005. THE REQUEST WAS GRANTED BY THE COMMISSION.

 

12)       Consideration of the following requests for authority, under the provisions of Civil Service Rule 6.16(c) for an Individual Pay Adjustment, as follows:  (Appearance by representatives of the agencies.)

 

a.         Department of Corrections for an Individual Pay Adjustment in the amount of 10% for Probation and Parole Officers that attain a master’s degree, effective May 4, 2005.  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

b.         Louisiana State University Health Sciences Center – Shreveport for an Individual Pay Adjustment for Jennifer Tonsman, Registered Nurse 2, in the amount of $38.40 bi-weekly, effective June 6, 2005.  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

c.         Department of Veterans Affairs for an Individual Pay Adjustment for Gustave Freche, Administrative Manager 5 at the Louisiana War Veterans Home, in the amount of $151.20 bi-weekly, effective June 15, 2005.  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

d.         East Jefferson Levee District for an Individual Pay Adjustment for James Baker, Levee Board Operations Superintendent, in the amount of $212.80 bi-weekly, effective June 15, 2005.  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

e.         Pontchartrain Levee District for an Individual Pay Adjustment for Kenneth Steib, Levee Forman 2, in the amount of $274.40 bi-weekly.  THE REQUEST WAS GRANTED BY THE COMMISSION, EFFECTIVE JUNE 15, 2005.

 

13)       Consideration of the following requests for authority to establish or amend an Optional Pay Adjustment policy under the provisions of Civil Service Rule 6.16.2 as follows:   (Appearance by representatives of the agencies.) 

 

a.         University of Louisiana at Monroe - to establish an Optional Pay Adjustment policy, effective June 8, 2005, as follows:  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

·        Retention of employees deemed essential to the agency – up to 10% base pay or lump sum increase to match a written and verified offer from a private employer, an unclassified position in state service, or a position at a non-state governmental entity.

·        Recruit employees into difficult to fill positions – up to 10% base pay or lump sum increase in order to recruit employees into difficult to fill positions.

·        Adjust pay differentials between comparable employees or employees within a supervisory chain – up to 10% base pay increase for salary compression caused by the implementation of either Civil Service Rules 6.5(b) or 6.5(g).

·        Compensation for additional duties – up to 5% base pay or lump sum increase to award employees for either permanent or temporary additional duties.  All increases above 5% for additional duties must receive approval from the Civil Service Commission before implementation.

 

b.         Office of State Examiner -  to establish an Optional Pay policy, effective June 15, 2005 as follows:  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

·        Retention of employees deemed essential to the agency for up to the maximum amount allowed by Civil Service.

·        Adjust pay differentials between comparable employees for up to the maximum amount allowed by Civil Service.

·        Recruit employees for jobs deemed difficult to recruit for up to the maximum allowed by Civil Service.

·        Compensation for additional duties of up to 5% for additional permanent or temporary duties.  Employees at range maximum pay receive a one-time lump sum payment of up to 5% of their base pay for additional duties.  Any requests for amounts over 5% and any requests for employees at the administrator level or higher must receive approval from the Civil Service Commission.    

 

14)       Consideration of the following requests for authority to establish or amend the Rewards and Recognition policy under the provisions of Civil Service Rule 6.16.1 as follows:   (Appearance by representatives of the agencies.)

 

a.         Louisiana State Board of Medical Examiners - to establish a Rewards and Recognition policy, effective May 1, 2005, as follows:  THE REQUEST WAS GRANTED BY THE COMMISSION.

 

·        Service Recognition – certificates, ribbons and/or service pins

·        Education and Training – monetary recognition not to exceed $1000 depending upon training/certification received

·        Innovation – certificates, ribbons, plaques, or monetary recognition not to exceed 9% of employee’s base salary

·        Initiative – certificates, ribbons, plaques, or monetary recognition not to exceed $50

·        Certified First Responder – monetary recognition not to exceed $250 for certification received

·        Employee of the quarter – certificates, ribbons, plaques, or monetary recognition not to exceed $50

 

b.         Office of Financial Institutions - to amend its Rewards and Recognition policy, effective June 15, 2005.  THE REQUEST WAS GRANTED BY THE COMMISSION.

a.         Department of Health and Hospitals for an optional pay adjustment of 5% for temporary duties in the amount of $148.80 biweekly for Edward Holmberg, Accountant Manager 4, effective July 1, 2005.  Additionally, the agency is requesting retroactive approval for an optional pay adjustment made in April 2005.  THE COMMISSION GRANTED AUTHORITY FOR AN OPTIONAL PAY ADJUSTMENT OF 5% FOR EDWARD HOLMBERT.  THE COMMISSION DENIED THE REQUEST FOR RETROACTIVE APPROVAL OF AN OPTIONAL PAY ADJUSTMENT MADE IN APRIL 2005.
