10)       Consideration of the following requests for authority, under the provisions of Civil Service Rule 6.16.2, to establish or amend the Optional Pay Adjustment policies as follows:  THE REQUESTS WERE GRANTED BY THE COMMISSION.           

            a.         Request of the Housing Authority of Shreveport to amend under the provisions of Civil Service Rule 6.16.2 its Optional Pay Adjustment Program Policy effective July 1, 2004 as follows: 

·        To amend its policy to remain in compliance with the guidelines established for additional duties. 

            b.         Request of the Department of Corrections, to amend under the provisions of Civil Service Rule 6.16.2 its Optional Pay Adjustment Program Policy effective July 1, 2004 as follows:

1)         Request of the Department of Public Safety, Office of Management & Finance, under the provisions of Civil Service Rule 6.16(a) for a one-time lump sum payment of $500.00 each to Mr. Daniel Taylor, Ms. Tammy Carter and Ms. Lona Gros for their attainment of the Certified Professional Public Buyer designation.  (Appearance by a representative of the Department of Public Safety, Office of Management & Finance) THE REQUEST WAS GRANTED BY THE COMMISSION. 

2)         Request of the Office of Public Health for authority under the provisions of Civil Service Rule 6.16(c) for an individual pay adjustment for Bertha LaNell Thompson-Washington, Public Health Nurse 5, in her base pay from $1742.40 to $2160.60 bi-weekly.  (Appearance by a representative of the Office of Public Health)  THE REQUEST WAS GRANTED BY THE COMMISSION EFFECTIVE OCTOBER 6, 2004. 

3)         Request of the Port of New Orleans for authority under the provisions of Civil Service Rule 4.1(d)2 for an extension of one unclassified position for Director of Business Development.  (Appearance by a representative of the Port of New Orleans)  THE REQUEST WAS GRANTED BY THE COMMISSION EFFECTIVE OCTOBER 30, 2004 THROUGH OCTOBER 31, 2007. 

4)         Request of the West Jefferson Levee District for authority under the provisions of Civil Service Rule 21.13, for an exception to Civil Service Rule 21.9 to pay overtime at a time and one-half rate to employees who are required to work beyond the regular workday/workweek in situations which the Levee District declares an emergency.  This request for compensation extends to non-exempt as well as exempt employees.   (Appearance by a representative of the West Jefferson Levee District)  THE REQUEST WAS GRANTED BY THE COMMISSION WITH THE CONDITION THAT A LIST OF ALL OF THE EXEMPT POSITION TITLES FOR WHICH THIS AUTHORITY WILL BE USED MUST BE SUBMITTED WITHIN 30 DAYS OF THE APPROVAL OF THIS REQUEST. 

5)         Request of the Greater Baton Rouge Port Commission for authority under the provisions of Civil Service Rule 4.1(d)2 for an extension of one unclassified Professional Civil/ Environmental Engineer position for an additional 3 to 4 years.  (Appearance by a representative of the Greater Baton Rouge Port Commission)  THE REQUEST WAS GRANTED BY THE COMMISSION EFFECTIVE NOVEMBER 9, 2004 THROUGH NOVEMBER 30, 2007. 

6)         Request of the Department of Health and Hospitals, Office of the Secretary for authority under the provisions of Civil Service Rule 6.16(c) for an individual pay adjustment for Ms. Erin Chisholm in order to keep her pay at $1443.20 bi-weekly, effective August 16, 2004.  (Appearance by a representative of the Department of Health and Hospitals, Office of the Secretary)  THE REQUEST WAS GRANTED BY THE COMMISSION EFFECTIVE AUGUST 18, 2004 THOROUGH SEPTEMBER 8, 2004. 

7)         Request of the Office of Mental Health for authority under the provisions of Civil Service Rule 6.16(c) for an individual pay adjustment for Mr. Gary A. Martin, licensed Pharmacist at the Mental Health Center of Central Louisiana, to bring his salary up to $2704.59 bi-weekly.  (Appearance by a representative of the Office of Mental Health)  THE REQUEST WAS GRANTED BY THE COMMISSION EFFECTIVE AUGUST 16, 2004. 

8)         Request of Office of Public Health for authority under the provisions of Civil Service Rule 6.16(c) for an individual pay adjustment for Ms. Jennifer Meyer, Program Manager 1, in her base pay from $1760.80 to $2062.33 bi-weekly.  (Appearance by a representative of the Office of Public Health)  THE REQUEST WAS GRANTED BY THE COMMISSION EFFECTIVE OCTOBER 6, 2004. 

9)         General Business Meeting No. 10: Addendum to the request for authority, under the provisions of Civil Service Rule 6.16.2, to establish or amend the Optional Pay Adjustment policies as follows:  THE REQUEST WAS GRANTED BY THE COMMISSION. 

            a.         Request of Louisiana Tech University to establish an Optional Pay Adjustment Program Policy effective October 6, 2004 as follows: 

·        Compensation for additional duties – up to 5% base pay or lump sum increase to award employees for permanent additional duties.
