9) Consideration of the following requests for authority, under the provisions of Civil Service Rule 6.16.2, to establish or amend the Optional Pay Adjustment policies as follows:
a. Department of Insurance – to revise its policy effective July 1, 2004 as follows:
To amend its policy to remain in compliance with the guidelines established for additional duties.
b. LSU Health Sciences Center - Shreveport – to revise its policy effective August 1, 2004 as follow:
To amend its policy to remain in compliance with the guidelines established for additional duties.
c. Secretary of State - to revise its policy effective July 1, 2004 as follow:
To amend its policy to remain in compliance with the guidelines established for additional duties.
d. Department of Environmental Quality - to revise its policy effective July 1, 2004 as follow:

e. State Plumbing Board of Louisiana - to revise its policy effective July 1, 2004 as follow:

g. Housing Authority of the City of Alexandria - to revise its policy effective July 15, 2004 as follow:
To recruit for difficult-to-fill positions up to a 7% base pay increase or lump sum payment.
Retention of employees deemed essential to the department up to a 7% base pay increase to match a written and verified job offer from a private employer to unclassified job offer.
Adjust pay differentials between employees in either the same job series or supervisory chain up to a 7% base pay increase only in the instance where a junior employee’s pay rate surpasses the pay rate of a senior employee.
A maximum of 5% will be awarded by the agency for the assignment of additional duties.
h. Louisiana School Employees’ Retirement System - to revise its policy effective July 1, 2004 as follow:
To amend its policy to remain in compliance with the guidelines established for additional duties.
i. Department of Public Safety - to revise its policy effective August 11, 2004 as follow:
To amend its policy to remain in compliance with the guidelines established for additional duties.

10) Request of the Housing Authority of the Town of Jonesboro, under the provisions of Civil Service Rule 6.16.1, to establish a Rewards and Recognition Program policy to reward employees with an effective date of October 1, 2004 in the following manner:
A one time payment of up to 8% of an employee’s annual salary for each year that their respective department obtains a "high performer" rating on the annual Public Housing Assessment System (PHAS).

11) Request of the Department of Transportation and Development, under the provisions of Civil Service Rule 6.16.1, to establish a Rewards and Recognition Program policy to reward employees with an effective date of August 11, 2004 in the following manner:
The award selection committee - committee members must be approved by an administrator.
The award nomination process – change in nomination deadlines, new stipulations regarding self-nomination of family members.
The amount of the "Secretary’s Award of Excellence" increase from $250 to $1000.
The way in which the award money can be used for the "Best Team Effort of the Year" award – hosting a Recognition Event for the Team instead of cash rewards.
Awards for performing additional safety-related duties – changes to duties that are eligible for award and training requirements for those with safety duties.

12) Request of LSU Health Sciences Center – Shreveport, under the provisions of Civil Service Rule 6.16.1, to establish a Rewards and Recognition Program policy to reward employees with an effective date of August 1, 2004 in the following manner:
Special individual achievement. This includes, but is not limited to perfect attendance, outstanding performance, etc.
Development of new procedure or solution, which results in cost savings and/or greater efficiency in operation.
Rewards may be monetary or non-monetary. If monetary, such rewards may range from $100.00 - $300.00, depending upon the level of accomplishment and/or impact to the institution.

13) Request of the Housing Authority of the City of Winnfield, under the provisions of Civil Service Rule 6.16.1, to establish a Rewards and Recognition Program policy to reward employees with an effective date of August 2, 2004 in the following manner:
A one-time payment of up to 9% of an employee’s annual salary for each year that their respective department obtains a "standard to high performer" rating within the first year of implementation and a "high performer" rating in subsequent years on the annual Public Housing Management Assessment Program.

1) Request of the Department of Health and Hospitals, under the provisions of Civil Service Rule 4.1(d)2, to establish an unclassified position of Liaison Officer at Southeast Louisiana Hospital. (Appearance by a representative of the Department of Health and Hospitals) THE REQUEST WAS APPROVED BY THE COMMISSION.
2) General Business No. 9 - Addendum to the requests for authority, under the provisions of Civil Service Rule 6.16.2, to establish or amend the Optional Pay Adjustment Policies as follows:
a) Department of Natural Resources - to amend its policy effective July 1, 2004 as follows:
To amend its policy to remain in compliance with the guidelines established for additional duties.
b) Louisiana State University - to amend its policy effective August 11, 2004 as follows:
To add a provision that optional pay may be used in any hire, but probational employees cannot receive an optional pay adjustment until after they attain permanent status.
To modify several special pay inequity definitions, including adding an "other" category and explaining the ability to adjust supervisory/subordinate pay inequities to allow for adjustments if the subordinate’s pay is within 7% of the supervisor’s pay.
To seek to establish a detailed additional duty program for one year to recognize new significant duties that fall short of justifying a reallocation. These adjustments are limited to 5% and are to be strictly interpreted and approved by the Executive Director of HRM.

5) Request of the Department of Wildlife and Fisheries, under the provisions of Civil Service Rule 21.13, for an exception to Civil Service Rule 21.9, to pay overtime at a time and one-half rate to positions within the Enforcement Division, with an effective date of July 1, 2004. 6) Request of Louisiana State University, under the provisions of Civil Service Rule 6.16.1, to establish a Rewards and Recognition Program policy with an effective date of August 11, 2004 in the following manner:
The University has decided to modify the CPTP awards to reflect that CST is no longer offered. The University will now award $125.00 for completion of each of the four individual management certificates in the state’s CPTP curriculum.
To remove the one-year waiting period and add the provision that professional and academic staff would no longer be eligible for the award.
